Transportation
Community Partnership on Aging and the City of Mayfield Heights offer transportation services through Senior Transportation Connection to residents age 60 and older who do not have another means of transportation. Registration is required to use the service. The Office on Aging can also assist with identifying other resources for transportation. Social Work Services
The Outreach/Social Work Department helps individuals and families cope with personal and family problems. Guidance is also offered for decision-making on lifestyle transitions. Counseling may be provided by our staff of licensed social workers or they may assist you in locating another agency to provide additional services. Seasonal Services
The following services are available in collaboration with other organizations at specific times of the year and are advertised in our monthly newsletter:
- AARP Tax Program - We are fortunate each year to partner with the AARP Tax Program in order to provide tax returns to residents. In order to receive this free service provided by trained AARP volunteers, persons 60 and over of low income are given priority. Tax returns are available by appointment only and limited to simple, ordinary, personal tax returns. Only copies of the e-file returns will be provided.
- Senior Farmers Market Nutrition Program - Each summer, CPA distributes coupons totaling $50 to eligible households to purchase fresh, locally grown produce. The program is funded by the USDA, the Ohio Department on Aging, and the Western Reserve Area on Aging. Please call our offices in late May each year to find out when the program will start.

Volunteerism
Attention Adults 60+: We offer a wide range of volunteer services including yard work, computer training, friendly visits, errand running, assistance with recycling and simple home repairs. Priority is given to client and/or individuals with financial need. Learn more about this service by calling 216-291-3903.
Produce Plus
Community Partnership on Aging partners with the Greater Cleveland Foodbank to offer Produce Plus, a monthly produce distribution program for older adults in Mayfield Heights and surrounding cities. Produce Plus takes place every third Thursday at South Euclid Community Center, 1370 Victory Dr., South Euclid. Currently, Produce Plus is offered by pick-up; eligible participants can drive up in their cars where CPA staff and volunteers will put the produce in your car’s back seat or trunk. Meals on Wheels
Community Partnership on Aging Outreach Workers provide information and registration for the Hillcrest Meals on Wheels program, a local nonprofit using a large volunteer corps to offer weekday delivery of meals. To schedule an appointment for registration, please call 440.442.2626 x 244.
Equipment Loan Program
If you need to borrow a walker, cane, bedside commode, or a number of other items, we've got them! They are clean and in great shape. Call 440-442-2626 x 244 to borrow or donate.
Safe at Home
The Safe at Home Program aims to enable low income older adults that reside in member cities to apply for and receive tools and equipment that will assist in adapting their homes for comfort and safety. The tools and equipment may include (but are not limited to):
- Grab bars for showers, baths and other transitional areas
- Shower chairs
- Hand-held shower faucets
- Hand rails
- Other items determined to be needed
The Safe At Home Program will be administered by a Community Partnership on Aging Social Worker and will include a home assessment as well as application with income and asset verification.
